OVERVIEW

Aneden Gives has developed an application and patient selection process to provide fertility grants for people desiring families in Seattle. Aneden Gives will select individuals and couples to receive treatment at Pacific Northwest Fertility. Aneden Gives will only disburse grant funds to the fertility clinic.

GRANT ELIGIBILITY CRITERIA

Applicants for the Aneden Gives Seattle Fertility grants must meet ALL of the following criteria:

1. Applicant's annual combined family income must not exceed $175,000, i.e., the combined Adjusted Gross Income on Line 11 of the 2024 IRS Tax 1040 must not exceed $175,000. 

2. Applicants must not have fertility insurance coverage. If applicants have fertility insurance coverage but have exhausted their coverage, they remain eligible. We are currently only accepting applicants who are patients of the Pacific Northwest Fertility (PNWF). 

3. Applicants must be legally allowed to live and/or work in the US. 

4. Only one application will be accepted for each family. Applicants applying with a spouse or partner must add them as a collaborator on the application to complete the Medical Release Form and Legal Consent. 

5. All applicants must sign an online Medical Release Form, consenting to providing Aneden Gives medical updates provided by PNWF and allowing Aneden Gives to contact the applicant(s) in the future. 

6. All applicants should be at least 21 years old.

GRANT APPLICATION FAQs

• If you are pre-selected for a grant award, the applicant(s) must sign an authorization for a Background Check. 

• All applicants will be asked to provide the following supporting documentation:

  • 2024 tax filings (IRS Form 1040) for all applicant(s)

GRANT AWARD TERMS

• Grant funds must be used within the first 12 months of the award. All funds are disbursed to PNWF directly.

• Submitting the online grant application means the applicants agree to share their stories and pictures with Aneden Gives. It also allows Aneden Gives to share their stories and pictures as we spread awareness about infertility and treatment options in Seattle. 

GRANT SCHEDULE

Grant Application Window: May 1 -  May 31, 2025

Application Review Period: June 1 – June 30, 2025

Pre-Selection and Applicant Notification Period: July 1 – July 16, 2025
